A critical step in the operative management of non-functioning asymptomatic incidentaloma.

چکیده
Incidentalomas are more frequently diagnosed now due to easy availability of radiological diagnostic techniques. These tumors pose a new challenge for development of therapeutic and preoperative evaluation strategies. The natural course of adrenal incidentalomas and the risk that such lesions lead to hormonal hypersecretion and malignancy are still under evaluation. Factors influencing transformation of a truly non-functioning incidentaloma into a hypersecretory or a malignant tumor are not entirely known. Surgery is recommended on the basis of functional type of the tumor and the potential for malignancy. Tumor size is being recommended to be the most reliable criterion for malignant potential. Surgery for adrenal tumors is fraught with perioperative complications unless the patient has been preoperatively prepared. The worst case scenario is seen in cases of pheochromocytoma, which are not preoperatively prepared as this case demonstrates. There are no clear recommendations in contemporary literature as to the pre-operative preparation of non-secretory, asymptomatic incidentaloma in normotensive individuals with normal biochemical and endocrinological profile.

Not functioning adrenocortical carcinoma from diagnosis to management: a case report
Abstract Adrenal gland cysts are rare indications of adrenal diseases, which are commonly asymptomatic. In radiological studies, these cysts, known as incidentalomas, are often detected by coincidence accounting for 6% of the population. Adrenal incidentalomas are commonly detected, and autonomous cortisol hypersecretion is the most prevalent abnormality associated with these masses. Since thi...

Introduction: Effects of high velocity bullet in Abdominal wall injuries specially the blast effect on intra abdominal organs needs mandatory explorative laparatomy to manage any major and minor injuries . Usually many explorative laparatomies are negative , but it can be reduced by advanced radiologic techniques such as ultrasonography ,CT – Scan , MRI preoperatively. In patients with negativ...

Background: Clavicle fractures are common and usually heal without complications. In this study, we evaluated the outcomes of non-operative versus operative management of displaced fractures. Methods: In a prospective clinical trial study, sixty-five patients with displaced clavicle mid-shaft fractures were nonrandomly divided in two treatment groups. The first group underwent non-operative...
